Service Description
Includes 4 weeks worth of medication, face to face consultation every 4 weeks, ability to contact me at any stage during those 4 weeks for updates or queries (message, phone call or face to face) medication and advise for management of side effects. Continuous guidance, and education in regards to Diet, Exercise, lifestyle changes and prevention of side effects. I also provide maintenance prescriptions.
Cancellation Policy
Thank you for booking your appointment! Please look out for a separate email containing a medical form and a consent form - these need to be completed prior to your appointment. If you don't receive this email within 24hrs from booking your appointment, please check your spam folder or contact me. Please note we require a minimum of 72 hours' notice for any appointment cancellations or rescheduling. Failure to provide adequate notice will result in a new deposit being required in order to reschedule your appointment. If notice is given at least 72 hours before the appointment, the deposit may be transferred to the rescheduled date. Treatment packages and gift cards must be used within 3 months from the date of purchase. Unused sessions beyond this period will expire and are non-refundable. Full payment is required at the time of service unless prior arrangements have been made. We accept cash, bank transfers and PayPal. See you soon!
